Innovation in Wakefield – Integrated NHS & Social Care Success

"Ferrybridge Medical Centre in Wakefield is a test area for new ideas. NHS & Social care are working together with 15 care homes, to provide support and reduce system pressures. Improvement in healthcare is achieved by treating the patient holistically and treating them as individuals.

Making the Care Homes feel supported by GPs and part of the NHS and Social Care systems improves patient care.

The activity savings have been identified in the period January - December 2016 as:

  • 19% reduction in emergency admissions
  • 12% reduction in A&E attendances
  • 26% reduction in NHS bed days
  • 9% reduction in ambulance call outs

Estimated potential savings for the Wakefield area are £5m. National funding has been received but it has been matched with local funding.
